Right coronary artery intervention is usually accomplished with a Judkins right (JR) guide catheter. Abnormal take-off from the sinus poses difficulty in engaging conventional JR We report rare case of primary percutaneous which was performed multipurpose catheter as patient had an extremely low-lying where catheters could not engage octogenarian acute inferior wall ST-elevation myocardial infarction cardiogenic shock.
